Replacing Lost Documents

Teudat Zehut

In order to issue a new Teudat Zehut you will need to visit your local branch of Misrad Hapnim (the Ministry of Interior). No appointment is necessary.

What you need:

  1. Valid photo ID (Israeli passport, Teudat Oleh, military ID, or Israeli marriage certificate – a driver’s license is not considered sufficient)
  2. Two identical passport-size photos
  3. 110 NIS
  4. Teudat Zehut Request Form
  5. Declaration of a Lost Teudat Zehut (available at Misrad Hapnim)

What you get:

  1. A new Teudat Zehut will be printed for you on the spot
  2. If you are unable to provide sufficient proof of identity, you will not receive your Teudat Zehut on the spot and instead Misrad Hapnim will check their records and then mail your new Teudat Zehut to your home address.

Important Note: If you need to replace a lost Sefach (addendum) and you have the physical Teudat Zehut with you, you can visit your local branch of Misrad Hapnim and issue a new Sefach free of charge. You will need to fill out a Teudat Zehut/Sefach Request Form as well as a Declaration of a Lost Teudat Zehut/Sefach.

Teudat Oleh

In order to issue a lost Teudat Oleh, you must first sign a declaration in front of a court clerk attesting to the fact that you lost the Teudat Oleh. On the form you will need to indicate any benefits which you may have used such as customs benefits or tax reductions. There is a 50 NIS processing fee for the court declaration. 

You will need to schedule an appointment at Misrad Haklita in order to issue a new Teudat Oleh.

What you need:

  1. Declaration signed by the court clerk
  2. Valid photo ID (Teudat Zehut, passport, etc.)
  3. One passport photo for each spouse

 What you get:

  1. Your new Teudat Oleh will be issued

Israeli Passport/Teudat Maavar

In order to request a new passport or Teudat Maavar you will need to visit your local branch of Misrad Hapnim. No appointment is necessary.

What you need:

  1. Teudat Zehut
  2. Two identical passport-size photos
  3. Request for an Israeli Passport/Teudat Maavar Form
  4. Declaration of a Lost Israeli Passport
  5. 1000 NIS (for a lost adult passport) or 500 NIS (for a lost Teudat Maavar or child’s passport)

What you get:

  1. Your new Israeli passport will be mailed to you, usually within 7 business days
  2. In certain cases Misrad Hapnim reserves the right to issue a Teudat Maavar (temporary passport) in lieu of a replacement Israeli passport

Driver’s License

There are three ways in which you can apply for a replacement driver’s license:

  1. Visit your local branch of Misrad Harishui. You can pursue this option if you don’t have any limitations on the license and provided that your license has not been suspended or revoked.
    1. What you need:
      • Valid photo ID (Teudat Zehut, Israeli passport, etc.)
      • 55 NIS
    2. What you get:
      • You will receive a temporary license/payment slip which you can pay at the bank or at your local post office.
  2. You may call Misrad Harishui’s automated phone system by dialing *5678 or 1-222-5678.
  3. You may apply for the new license and pay via the payment site on www.gov.il.

Please note: If you opt to apply by phone or online, your temporary license will be sent to you by mail. This process is not recommended if you need the new license urgently. The temporary license is valid for several months. Your permanent driver’s license will be mailed to your home address.

Bank Form 

If you lose the “Note of Future Bank Account” which you received with your Teudat Oleh at the airport, there is no need to replace it.  Instead, when you open your bank account, make sure to ask for an opening statement which lists your personal details and account number. Present the opening statement to your local branch of Misrad Haklita in order to arrange your Sal Klita payments. 

Kupat Cholim Registration Form 

There are two ways to replace your Kupat Cholim Registration Form:

  1. Request a new form from your Misrad Haklita counselor
  2. Request a registration form from your local post office

Please note: If you do not register at the post office within 90 days of your Aliyah, you will need to visit your local branch of Bituach Leumi in order to register for health coverage.
