Required Documentation

Applicants for recognition in Chiropractic must submit the following documents:

1. Questionnaire for Health Care Professionals

  • Must include an Israeli address and cell phone number (this may belong to friends or family).

2. Identification Document

  • A copy of your Israeli ID card, including the page showing your address (if applicable).
  • If you do not yet hold an Israeli ID, submit a copy of your valid foreign passport.

3. Final Diploma or Certificate of Completion

  • From a recognized chiropractic institution.
  • Must be authenticated by an Israeli notary.
  • If issued in a language other than English or Hebrew, it must be translated into one of those languages.

4. Professional Opinion

  • This must certify your supervised work period of six months (20–40 weekly hours, minimum three days per week) in a hospital or clinic in Israel where chiropractic or spine-related services are provided.

5. Permit to Work Under Supervision

  • Before beginning supervised work, you must obtain an official permit.

  • Submit to the Medical Professions Licensing Division, Ministry of Health, 39 Yermiyahu Street, P.O. Box 1176, Jerusalem 9101002.

6. Certificate of Good Standing

  • Issued by the relevant licensing authority in your country of registration.
  • Must confirm that no disciplinary or ethical complaints exist.
  • Should be sent directly from the licensing board to: [email protected]
