All NCCPA registered physician assistants are able to submit their documents to the Ministry of Health, and their files are eligible to be reviewed to receive an Israeli license. We recommend that you begin gathering your documents for submission. For applicants who are not registered with the NCCPA, please click here.

Required Documents

  1. Medical Licensing Questionnaire
    This is an online form. Once you’ve completed the form, it will automatically download onto your computer. If you’re physically submitting your documents, please print the form out. If you’re submitting your documents electronically, please save the file and make sure to include it in your online document submission. Important: Make sure to have your legal name written in English AND in its Hebrew transliteration (DO NOT use your Hebrew name). In addition, you must provide an ISRAELI address and cell phone number (can be a phone number of friends/family). Your file will NOT be opened without this information.

    If you’re already an Israeli citizen and have an Israeli national ID card, please attach a photocopy of the card, as well as a photocopy of the card’s paper addendum.
    If you’re an Israeli citizen and DO NOT have an Israeli national ID card, please include a photocopy of your Israeli passport.
    If you are applying pre-Aliyah, please submit a photocopy of your foreign passport.

  2. Final diploma*
    or official confirmation of eligibility for your diploma provided you haven’t yet received a copy of the diploma.
  3. Specialist’s certificate from abroad (If applicable)
  4. Professional letter of good standing
    From the board of the state in which you are licensed. The letter must be sent directly from the Board of Medicine to the Ministry of Health. If the letter can be sent electronically, please have it sent to [email protected]. For further clarification regarding this requirement, please be in touch with us at [email protected].
    Please note that this document has limited validity.
  5. Official documentation
    – indicating a legal change of name, if relevant.
    – proving that you’ve completed 1500 required hours of training.*
    – NCCPA board certification.*
    – detailing your employment history, specifically noting that you’ve been gainfully employed for at least 2 out of the last 5 years.*
  6. Personal statement describing your professional/clinical work experience over the last few years.

Please Note:

This is only a required document if you haven’t retaken the NCCPA exam in the last 10 years.

Some of the documents listed above are only valid for one year from their issuing date. If you have not submitted your Teudat Zehut (ID) within a year of opening your file with the Misrad Habriut, you may be required to present valid, re-issued documents in order to request your temporary license.

Do not send original certificates. Send copies and retain original documents, as the Ministry of Health will not return or duplicate them.

Double-check that you’ve provided accurate contact details. Incorrect information will delay processing.

Documents that are not in Hebrew or English must be translated into Hebrew and notarized. Ensure that you provide two copies of each document: the notarized version and another copy.

Verification of Documents

For the documents that require verification, you have 3 verification options:

  1. Have a notarized copy of the document (notarized by a local notary-public) AND have that copy apostilled. Please be in contact if you need help or further clarification.
  2. Bring it to an ISRAELI notary for authentication. (Digital notarization by an Israeli notary is also acceptable).
  3. Set a consular services appointment at your local Israeli consulate and request that they issue a verified copy (אימות העתק, Imut He’etek) of the document.

Keep your original documents. Submit only verified COPIES.

Receiving your license

Your permanent license will be issued after you’ve officially completed your Aliyah, and it will be sent to the Israeli address you provided.

Payment will be requested via an online payment service. You’ll receive an SMS to the Israeli number you provided with the relevant link.

If you need to update your Israeli number or address, please let us know and we will pass the information forward.

Please note: You can complete this entire process at MedEx NJ.
If you are a licensed PA from outside of the USA, please feel free to open a file on the Ministry of Health Portal and begin submitting your documents as detailed above. The licensure process for those licensed outside of the USA is still being finalized, and therefore will likely take more time. There are also still discussions regarding a potential licensing exam. Misrad Haklita Reimbursement for Documents

The Ministry of Aliyah and Integration is offering a retroactive reimbursement of up to 4000 NIS to Olim who were required to translate and/or notarize documents for the purpose of transferring their professional license in Israel. Please contact your local Misrad Haklita office for more information.

Please note the following conditions:

  • This can be done ONLY AFTER completing your Aliyah.
  • The reimbursement is only retroactive for 2 years prior to your date of Aliyah.
  • Only Olim who have not yet reached retirement age are eligible.
  • Reimbursement can be made only for Israeli notary/consulate verification with original receipts (or verified copy) . Apostille is not reimbursable!

In addition, Misrad Haklita offers a reimbursement (up to 500 NIS) for Olim who took the governmental licensing exam. Your eligibility lasts for 10 years from your date of Aliyah. The reimbursement is ONLY given retroactively and will be paid back ONLY after submitting the receipt for your exam to your local Misrad Haklita office. Please contact your local Misrad Haklita office for more information.

Please note:  If you didn’t participate in a Misrad Haklitah preparation course for the governmental licensing exam, you may be entitled to reimbursement for two exams (if needed).

Hebrew Proficiency

Please note that you must demonstrate Hebrew proficiency in order to receive your license. You can do so in one of the following ways:

  • Passing the licensure exam in Hebrew

  • Passing the Bagrut exam in Hebrew (average score of 60).

  • Successfully completing Ulpan Gimmel at a Ministry of Aliyah and Integration approved Ulpan (score of 70 or higher on the final test).

  • Pass the NITE-YAEL exam (score of 65 or more) – the exam is offered several times a year, for a fee, at various locations in Israel.

  • Complete an advanced Medical Hebrew Ulpan offered by the Ministry of Aliyah and Integration as a new Oleh

Non NCCPA registered applicants

In addition to following the steps and document submission outlined above, it is important to note that if a PA is only registered with CAPA – ACAM, even if they trained and have a PA degree from the US, they should expect to sit for a licensing exam.
