PASSPORT FORMS: All DS-11, DS-82, or DS-5504 passport applications MUST be filled out online at this link https://pptform.state.gov/

and PRINTED. DO NOT fill out an application by hand, and DO NOT use an electronic application obtained from a source other than what is provided at the link.

DELIVERY: All applications for passports and CRBAs to be delivered in Israel MUST include a pre-paid Israel Post Courier slip per this link: https://israelpost.co.il/embassy

The cost of the courier service from Israel Post is 60 NIS – only one label is required per family.  After making the payment online, please print out the barcode confirmation and bring it to the interview. (Aramex delivers to the West Bank and Gaza – not to Yehuda V’Shomron.)

New Appointment System for Passport, CRBA (in-person and video), and Notarial Services:

Please do not pay any sums of money to third parties – our appointments are and have always been free.  There are no vendors associated with the U.S. Embassy, and you do not need them to make appointments or fill out forms. We urge you to make appointments yourself.

All new appointments for Jerusalem and Tel Aviv will be released in the new system.  This includes passports, CRBA, and notary services.  Additional appointments will be added each week on Wednesdays between 3:00 – 4:00 pm.

Please follow this link to create a profile and make an appointment in Jerusalem or Tel Aviv for passports, Consular Reports of Birth Abroad, and notary services.

In addition to the new appointment website, the vendor will have a customer service call center to assist applicants with basic questions regarding passport, Consular Report of Birth Abroad (CRBA), and notary services.

DROP BOX: IMPORTANT*** As of December 1, 2025, drop box service is changing.  Jerusalem is offering a drop box window service for DS-82 passport renewal and DS-5504 passport replacement applications.  AN APPOINTMENT MUST BE MADE on the US embassy appointment page  for all DS-82 and DS-5504 applications dropped off in Jerusalem (no appointment necessary for Tel Aviv).

All other drop box services offered in Jerusalem and Tel Aviv will remain unchanged.  Mail-in service for both DS-82 and DS-5504 applications remains available for both locations. https://il.usembassy.gov/american-citizen-services-drop-box/
