📍 Jerusalem (Full-Time / Option for Part-Time)
📍 Mercaz (Part-Time, 50–70%)

At B’lev Shalem, we provide personalized care management services in Israel, supporting seniors and their families — often based abroad — with hands-on care coordination, advocacy, and guidance.

Our work supports families through some of the most challenging and significant stages of aging with compassion, professionalism, and continuity.

We’re looking for experienced, compassionate Care Managers to join our growing team.

This is meaningful, relationship-based work for someone who:
• Loves working with seniors
• Is highly organized and proactive
• Can navigate complex medical and social systems in Israel
• Seeks autonomy, purpose, and truly impactful work

The role includes:
• Conducting in-home assessments
• Developing and overseeing individualized care plans
• Coordinating medical care and home support
• Navigating Israeli healthcare and social service systems
• Supporting families through crises and transitions

What we’re looking for:
✔ Background in gerontology, social work, nursing, OT, or related field
✔ Minimum 2 years’ experience working with seniors in Israel
✔ Fluent English + Hebrew proficiency
✔ Strong interpersonal and organizational skills
✔ Ability to work independently and manage multiple cases
✔ Comfort using cloud-based applications

Preferred:
• Experience in geriatric care management in Israel
• Familiarity with Bituach Leumi, Kupot Holim, and foreign caregivers
• Experience working with seniors with dementia
