based abroad \u2014 with hands-on care coordination, advocacy, and guidance.<\/p>\n<p>Our work supports families through some of the most challenging and significant stages of aging with compassion, professionalism, and continuity.<\/p>\n<p>We\u2019re looking for experienced, compassionate Care Managers to join our growing team.<\/p>\n<p>This is meaningful, relationship-based work for someone who:<br \/>\n\u2022 Loves working with seniors<br \/>\n\u2022 Is highly organized and proactive<br \/>\n\u2022 Can navigate complex medical and social systems in Israel<br \/>\n\u2022 Seeks autonomy, purpose, and truly impactful work<\/p>\n<p>The role includes:<br \/>\n\u2022 Conducting in-home assessments<br \/>\n\u2022 Developing and overseeing individualized care plans<br \/>\n\u2022 Coordinating medical care and home support<br \/>\n\u2022 Navigating Israeli healthcare and social service systems<br \/>\n\u2022 Supporting families through crises and transitions<\/p>\n<p>What we\u2019re looking for:<br \/>\n\u2714\u00a0Background in gerontology, social work, nursing, OT, or related field<br \/>\n\u2714\u00a0Minimum 2 years\u2019 experience working with seniors in Israel<br \/>\n\u2714\u00a0Fluent English + Hebrew proficiency<br \/>\n\u2714\u00a0Strong interpersonal and organizational skills<br \/>\n\u2714\u00a0Ability to work independently and manage multiple cases<br \/>\n\u2714\u00a0Comfort using cloud-based applications<\/p>\n<p>Preferred:<br \/>\n\u2022 Experience in geriatric care management in Israel<br \/>\n\u2022 Familiarity with Bituach Leumi, Kupot Holim,
