Entry and Exit Forms
Please submit a detailed list of all of your entries and exits from Israel for the seven years prior to your anticipated Aliyah date (using the Entry Exit form available for download on your Application Status webpage).
Please note the following exceptions:
- If you were born in Israel or lived in Israel as a child, and left before your 14th birthday, please list your entries and exits from your 14th birthday until the present.
- If you previously held an A1 visa, or made Aliyah in the past, please list your entries and exits from the date you left Israel until the present.
- If you are a returning Israeli citizen (Toshav Chozer), please list your entries and exits for the two years prior to your anticipated return date.
Entry and Exit (Father)
Please list your father’s entries and exits to Israel on the attached form. The entries and exits need only pertain to the time when you were between ages 13-17 or 14-18; this form must be signed by your father.
Entry and Exit (Mother)
Please list your mother’s entries and exits to Israel on the attached form. The entries and exits need only pertain to the time when you were between ages 13-17 or 14-18; this form must be signed by your mother.
