In cases of separation or divorce, where you have minor children, have your ex-spouse complete and sign this declaration (available for download on your Application Status webpage).
The signed declaration must be notarized, and in the event that you have minor children making Aliyah, the declaration must be certified with an apostille.
Alternatively, the declaration may be certified by your local Israeli Consulate.
